UW-Madison said Wednesday that it shut down a fraternity for the first time since 2006 after learning of a hazing weekend in December that included a freshman pledge suffering a concussion when an older member hit his head accidentally as the freshman lay in a closed crate meant to simulate a grave, a university official said.

The 21-year-old, who graduated from Oshkosh West High School in 2012 and now attends the University of Wisconsin-Madison, has competed in seven pageants during the past four years, and that experience finally paid off as she took home her first crown.

The investigations are being handled by the U.S. Department of Education’s Office of Civil Rights. Federal investigators have asked UW-Madison for all of their files regarding sexual assault cases and policies from 2012 to 2014. Investigators are trying to get a better sense of how the university handles sexual violence cases on campus.
